Dental Radiography Clinical Sign-up Form

You must have completed all of the theory work in the learning management system (LMS) prior to attending your chosen clinical sessions or you will not be able to participate. You must also be prepared to take an in-person theory test on the first day of your scheduled clinical sessions.

If you do not attend your scheduled clinic sessions or do not pass, you cannot retake the clinical sessions and you will be required to pass our DENT 9284 Dental Radiography: HARP Safety and Compliance Refresher course.

Clinical sessions run over two consecutive days. You must attend the clinical sessions you register in. Our facilities have a limited number of seats, and rescheduling cannot be easily accommodated.

• For sessions that start on Fridays, the Friday class runs between 6 p.m. and 9 p.m. and the Saturday class runs between 9 a.m. and 5 p.m. 
• For sessions that start on Saturdays, the Saturday class runs between 9 a.m. and noon (12 p.m.) and the Sunday class runs between 9 a.m. and 5 p.m.
