This form has been designed to assess whether or not you are eligible to have your course fee covered by the Ontario government’s Nursing Program Transformation in Ontario’s Colleges initiative. The following courses are part of this initiative:

This form is only for internationally educated nurses (IEN) who have been assessed by the College of Nurses of Ontario (CNO) as needing additional education to meet the competencies required to become registered in Ontario as a general class registered nurse (RN). To submit the form, please complete the following.
