Our Health and Transitions – NTI course explores the concept of health in Canada while focusing on the impact of health transitions on various stakeholders in different circumstances and contexts. Transitions in health occur often during the lifespan of individuals, families, groups, and communities. Health care transitions involve assessing, planning, and co-ordinating effective continuity of care for clients who experience a change in health status, provider, or location or who require support to manage their ongoing health needs. Learn to support and advocate for clients during their health and wellness journey using strength-based and health-promoting practices.
