Instructions for the Toronto Police Service Check Application Form

Important Note

  • As of January 1, 2015, the Toronto Police Service (TPS) fee rate has increased to $20 plus the cost of your money order fee.
  • You must be a resident of the Toronto region with a postal code that starts with the letter “M”.
  • Act now, as it takes 4 to 8 weeks or longer to process and receive your result.
  • It is your responsibility to make sure that you receive your clear police check prior to the deadline. Otherwise, you will not be allowed to participate in your placement, which may jeopardize your academic status in the program.
  • If you lose your original police check report, the Toronto Police Service will not send you another copy. You will need to apply again and pay another $20 fee.

How Do I Apply for or Obtain the Toronto Police Service Check Application Form?

Step 1

Pick up the application form from the Clinical Pre-placement Office, either in person or by booking an appointment.

Step 2

Fill out, complete and sign Sections 1, 2 and 3 of the application form. Make sure that your writing is legible.

Step 3

Bring a money order/bank draft payment of $20 made payable to Toronto Police Service and a government-issued photo identification card. Keep your receipt as your proof of payment. (We do not accept cash, credit cards or personal cheques).

Step 4

Submit your TPS form and payment either in person or by mail, or leave it in the drop box:

Step 5

Once received, we will mail your application form and payment directly to TPS on your behalf within 24 hours.
