Continuing Education Clinical Pre-placement Health Form Requirements
Continuing Education students currently registered in certain Community Services, Health Sciences and Nursing certificate programs may need to complete a clinical pre-placement health form for their clinical/field placement. This health form requires proof of immunizations, laboratory (blood test) results, CPR certification and mask fit testing. A vulnerable sector police check and standard first aid may also be required, depending on the program.
IMPORTANT ANNOUNCEMENTS February 9, 2010 Effective February 1, 2010, if you need to visit ParaMed for a third time, you will be charged a fee. You must pay the fee at your nearest Student Services Centre. The Clinical Pre-placement Office will send you a letter with more details. If you fail to pay, a hold will be placed on your student account.

- For students who reside in Toronto: If you need to apply for or renew your vulnerable sector police record check and you currently reside in the Toronto region (your postal code starts with an M), you must meet with Suzette Martinuzzi, Pre-placement Health Services Assistant, in order to receive a copy of the Toronto Police Services vulnerable sector police record check application form and have it signed by an authorized College representative. To book an appointment with Suzette, call 416-415-5000, ext. 3415, or e-mail smartinu@georgebrown.ca. You must bring a driver’s license or another form of government photo ID to your scheduled appointment. Once it is completed and signed, you can submit the form to the Toronto Police Service. Please be advised that it could take up to six weeks for the police service to process your application.
- For students who reside in another region or province: If you live in another region or province, check your specific regional police services web site (for example, Durham, Halton, Peel or York) and read their instructions carefully to learn about their requirements and the process for obtaining their application form.
